All our placements start with a risk assessment. This assessment is used to determine the needs of the young person, and the possible risks around the young person to identify the best possible semi-independent placement unit for them. This assessment is conducted by a team of experienced care managers.
The team considers the following factors when making a placement decision:
The young person’s needs and goals
The young person’s strengths and weaknesses
The availability of programs and services
The young person’s preferences
The risks around the young person.
Once a placement decision has been made, the young person is assigned to a designated staff member. This staff member will work closely with the young person to help them achieve their goals. The staff member will provide the young person with education, skills training, and lifelong learning opportunities. The staff member will also provide the young person with support and guidance as they transition to independent living. These will be delivered through key sessions twice a week at a time when the young person is available at reoccurring days and times.
